Procedure or Surgery Care.
Many surgeries and procedures are done on an outpatient status, meaning the patient will return home the same day. This is wonderful for the patient who gets to relax and recover at home; however, it can be overwhelming to return home by yourself with new medical interventions or with family members who feel uncomfortable and are unable to care for you properly.
Family members often have to take time out of their schedules to help loved ones with these procedures. Sometimes, loved ones are not available. Nurse Connect is here to help you not feel alone. Our nurses will bring you to your scheduled procedure, wait with you in the Pre-Operative area, and be present with you in the Post-Operative area where we will ensure proper discharge information and instructions are understood. Then when you get home and are more aware after your procedure, your nurse will go over all the information from the procedure/surgery with you and ensure you are confident and empowered to care for yourself.
You may be prescribed medication by your surgeon or provider, and we can assist in picking these medications up for you either at the hospital before leaving or through a drive-thru pharmacy on the way home. We will ensure you are comfortable at your home, Airbnb, hotel, or wherever you choose to recover. Your Nurse Connect nurse will educate you about your procedure, go over discharge information, and help you understand medications you need to take and when to take them. Your nurse will also check your vital signs after returning home from your procedure. Please be advised that all hospitals and surgery centers require a designated individual to monitor you for a period ranging from 12 to 24 hours post-procedure. When you engage with our services, we will assist with transportation to and from the procedure and provide monitoring for the specified duration signed in the Client Agreement form.
If nursing care is required for 12 to 24 hours following the procedure, you should arrange and discuss this with us in advance. Nurse Connect nurses will not complete any hospital forms indicating responsibility for your care beyond the agreed-upon timeframe unless explicitly contracted for that duration.
In the event that another caregiver will assume responsibility from the Nurse Connect nurse, a hand-off of care form must be signed at the time of transition. Please note that Nurse Connect will not be liable for any cancellations of surgeries/procedures resulting from not following hospital instructions on home monitoring post procedure.
