Patient Advocacy.

If you or your loved one has medical needs and are not medically savvy, we are here to provide the knowledge and time necessary to assist in these appointments.

We will obtain a medical history from the patient or their caregiver before the expected appointment date and attend the appointment with you to ensure the patient and their caretakers understand what is discussed and ask questions as needed. Our nurses can also transport the patient to and from their appointments. Then we will debrief, educate, and communicate healthcare goals with the patient and family as needed.

If follow-up appointments are needed, we can assist in scheduling. If medications are needed, we can assist in picking up medications and helping patients organize their medications while giving guidance on care planning going forward.
